The Oregon Health Plan (OHP) is an insurance plan that offers medical, dental, and/or mental health coverage at little or no cost to Oregonians.

To apply for OHP call (1-800) 359-9517 or TTY: (1-800) 621-5260 to receive an application packet.

Oregonians over the age of 60, disabled, or who already receive Medicare can also contact their local Department of Human Services (DHS) office for Seniors and People with Disabilities, or the local Area Agency on Aging for more information.

Oregonians who want individual coverage or coverage for their family should contact their DHS Self Sufficiency Office.

Oregonians who have a foster child and would like information about OHP should contact their DHS Child Welfare or Oregon Youth Authority worker for information about OHP.
